Enhancing Mortality Data Quality- Utilization of ICD-10 for Accurate Cause-of-Death Documentation in Form-4 for Death Registration

Abstract This study examines the application of ICD-10 (International Classification of Diseases, 10th Revision) for accurate cause-of-death documentation in Form-4, a standard form used for death registration. By utilizing ICD-10, medical personnel aim to improve the accuracy, consistency, and comparability of mortality data. This report investigates challenges in ICD-10 implementation, evaluates its advantages and disadvantages, and explores how ICD-10 impacts the quality of public health data. Key findings suggest that while ICD-10 coding enhances mortality data quality, implementation obstacles such as training and documentation burden persist. Recommendations include enhanced training programs and better integration of ICD-10 into healthcare systems to improve data accuracy.
